Carers Outreach will provide short break opportunities for adult carers across Anglesey, Conwy and Gwynedd. The project will be delivered by a dedicated Officer to support carer applications and a Community Officer to target and support underrepresented carers, and older carers in rural areas to access short break opportunities tailored to their needs.
Carers can request the funding for a break of their choice (up to £300).
Carers can make contact via telephone, email or through the organisations website to request funding. The Carers Support Officers will speak to carers to discuss their wishes and support the submission of an application on their behalf.
